There are four options of model release forms that will outline the terms of utilization of the Work(s) that you may view before booking. If you are unsure of which form will apply to your booking, please email in advance, and for all questions, concerns or clarifications of terms. These forms serve as legal documentation of our business transaction, documentation of your photo identification & date of birth, as is required for verification that you are of legal age to consent to the Work(s), and so that all parties are clear on terms prior to payment deposit & booking confirmation.

Neither of us should say we didn't understand what the other was thinking or needed, so let's get clear y'all...CONSENT!

All clients are required to sign an electronically delivered waiver at each instance & time of booking and provide a copy of proof of identification (DOB & legal name needed only).
